What should be included with the Form R?
Refer to the check list on back of the Form R for a list of items to include with the packet.
What items need to be completed on the Form R?
The type of appointment (i.e. new appointment) needs to be marked, the complete name of the candidate, the department, the individual's present rank, date that they were appointed to that rank, proposed rank, proposed salary, does the position depend on the availability of outside funds, effective period of appointment beginning and ending dates, as well as approvals or disapprovals and signatures by the appropriate individuals per the Form R. Remember to check the items on the back of the Form R that are included in the packet.
What happens if a Form R is incomplete?
If a Form R is incomplete, the appointment is delayed and the offer letter cannot be sent to the candidate.
